RN - Intake Coordinator

AccentCareHome Health of Rogue Valley has an immediate need for an INTAKECOORDINATOR in our Medford, OR office. 

            With direct report to the agency Administrator, the Intake Coordinator will receive, screen and process all referral/intake information including the receipt and accurate recording of clinical orders and demographic information, distribute information to appropriate team members, track and log intake/referrals in Allscripts, and triage clinical calls from staff, patients, and physicians.

Duties:

 

1.      Receives, screens and processes all referral information to the appropriate team member.

2.      Communicates all referral/intake information to appropriate Scheduler and Clinical Coordinator or Clinical Supervisor immediately upon intake.

3.      Coordinates referral with Scheduler to ensure quality and appropriate patient services.

4.      Receives and processes calls requesting information about AccentCare Home Health. 

5.      Makes every effort to provide services as appropriate to customers at the time of the initial call.

6.      Complies with State and Federal regulations and practices.

7.     Inputs Referral information into Allscripts while taking initial calls for service.

8    Participates in in-service education programs, and other agency meetings as required.

9.    Assists with Data entry when applicable; Medication Profiles, etc.

10. Processes PT/INR results with physicians' offices and patients.

11. Triages calls from clinicians, patients, families, and physicians' offices.

Qualifications:

 

Knowledge/Skills/Abilities:

?          Requires a valid Oregon license as a Registered Nurse.

?          A minimum of one year experience as an RN in home health.

?          Knowledge of state and federal home health requirements.

?          Data entry skills, excellent verbal and written communication skills (especially telephone etiquette).

?          Demonstrates good organizational skills and the ability to prioritize and multi-task. 

?          Uses professional judgment in reporting and seeking assistance from both peers and supervisors.

?          Superior Customer Service skills.
